Signing Up

Do you need to sign up to administer the ASPS? Call our toll-free helpline at 1.888.841.7536. You will be automatically connected with the technical advisor in your area who will walk you through the sign up process, ship you your survey materials, and answer any questions you may have.

Use the Survey as a Pre/Post or as a once-a-year behavior survey
The ASPS Survey can be used by grantees as either a Pre/Post survey that gathers information on change over the year or as a once-a-year evaluation of Positive Behavior Change and protective factors.
If you are using the ASPS once-a-year we recommend you use the Exit survey.

What do you get from the ASPS?
A tested survey that looks at both positive behavior change and student connectedness to their school and program. After administering the survey each grantee receives an ASPS Grantee Report as well site-level Reports that outline responses and levels of connectedness. As the ASPS data aligns with CHKS data, the Grantees also receive local district reports that compare CHKS results by students that attend ASPs and those that don't.

MOU Ready for 2007-08 School Year
The Memorandum of Understanding (MOU) for the 2007-08 school year is now available for download on this site.

Each grantee conducting the ASPS is required to sign a Memorandum of Understanding (MOU) on an annual basis. The MOU outlines the responsibilities of both the grantee and the HKS Regional Center and the conditions that must be met in administering the survey. Grantees wishing to administer the ASPS must sign and return this form to the HKS Regional Center via mail or fax, and follow all administration instructions in the ASPS Guidebook.
