Download the After-School Program Survey (ASPS)


The After-School Program Survey consists of an Intake survey which is administered when youth enter a program at the beginning of the year, and an Exit survey given at the end of the year. These two surveys act as a pre- and post-test, giving after school programs a method to measure the impact their program has had on youth over the course of the year.

Note: The After-School Program Survey(ASPS) is administered in after-school programs. If you are looking for the District After-School Module (DASM) of the California Healthy Kids Survey, which is administered in school, it is available for download on the CHKS Survey Download Page.
After-School Program Survey Intake Survey (ASPS-I)
To be administered to program participants at the beginning of each year as a "pre-test."
After-School Program Survey Exit Survey (ASPS-E)
This survey can be administered as either
an Exit survey,
Or as
a Once-a-year ASPS survey, to assess annual outcomes.
