Active Consent
Active consent requires written permission from a parent or guardian before any child can take the survey, and is required for grades below seven. It provides extra protection against surveying a child whose parents did not receive notification or did not approve of participation but failed to inform the program. A sample form is provided here in modifiable format. For information regarding consent, please review the Guidebook, above.

Passive Consent
Grantees administering the survey to grades 7 and higher are eligible to use passive parental consent, rather than active consent as previously required. Under passive-consent procedures, parents/guardians inform the program only if they don't want their child to participate (called "opting out"). A sample form is provided here in modifiable format. - Proctor Instructions
- The Proctor Instructions are designed to help train survey administrators (called proctors) on all administration and data collection procedures. Instructions include an Assurance of Confidentiality Agreement for proctors to sign and an Introductory Script to read to participants before each survey.

- 21st CCLC Memorandum of Understanding (PDF)

Each 21st CCLC grantee conducting the ASPS is required to sign a Memorandum of Understanding (MOU) on an annual basis. The MOU outlines the responsibilities of both the grantee and the HKS Regional Center and the conditions that must be met in administering the survey. Grantees wishing to administer the ASPS must sign and return this form to the HKS Regional Center via mail or fax, and follow all administration instructions in the 21st CCLC Guidebook.
