Customize a Module
Did you know you are able to add a series of questions to the CHKS? This can be done by having a custom module developed (Module G).
Questions can refer to a topic, a program or, in fact, almost anything you want information on. It's a simple way to get data on something unique to your school or district.
Search below for a current list of all questions asked in the various Module Gs across California.

Tips on creating a custom module:
- Custom modules cannot be longer than 42 questions, however, we recommend that they be less than 20.
- Students can only select one answer per question. "Mark all answers that apply" style questions are not allowed.
- Avoid using the word "or" and do not use "etc." after e.g. or i.e.
- Adjust the wording to suit the grade level.

Please note: the questions listed are those that have been used in the various custom modules to date. Please use these questions as a guide only. WestEd is not making any claims to their utility and clients wishing to develop a custom module should do so in consultation with their Regional CHKS center.
