Parents.

Different versions of the Healthy Kids Survey are administered at each grade level, some of which are separated into sections, called modules. Your district may not be administering all of the surveys or modules available on this site, and may be administering additional questions not on this site and only available through the district.
Please view a copy of the surveys at your child's school office, or contact the survey coordinator listed on your survey notice to determine exactly which questions will be administered at your child's school.
