Custom Modules

2009-10 Versions

The custom module option - sometimes called "Module G" due to its placement on the secondary answer sheet - allows LEAs to create a survey section that targets areas of specific interest.

These modules is especially useful for LEAs that want a certain program or health issue evaluated.

Listed below are some of the larger custom modules.

Note: Schools and districts need to inform their Regional CHKS Center if they plan to: administer a Custom Module; create their their own Custom Module; or plan to do more than one Custom Module.
