Memorandum of Understanding (MOU)

Note: This MOU is for on-cycle surveys (once every two years); call your HKS Regional Center if your district is surveying in an off-cycle year.

Memorandum of Understanding (PDF): Each California LEA that administers the HKS is required to sign a Memorandum of Understanding (MOU) that includes a Confidentiality Agreement. The MOU outlines the responsibilities of both the LEA and the HKS Regional Center and the conditions that must be met in administering the survey. LEAs wishing to administer the HKS must sign and return this form to the HKS Regional Center via mail or fax, and follow all Sign-Up Instructions.

Note: submission of this form alone does not sign you up for the survey. You must also make contact with your Regional CHKS Center after they have received the MOU.
Memorandum of Understanding for Requesting Raw Data (PDF): This MOU outlines the responsibilities of both the LEA and the HKS Regional Center for a request of the LEA's Raw Data Set. Raw data is provided in SPSS format for further data analysis. Familiarity with SPSS and/or data analysis is required. This allows districts or counties to further analysis.
