Please note: A series of questions about Physical Activity, Nutrition and Health has been moved from the required Module A to the voluntary Module E. These 10 items have been placed at the beginning of Module E. If your school/district has been using these questions and wish to continue they should direct their schools to also administer the first 10 items of this Module.
